Haven't extensively played with every piece of gear & shit, but it seems that pre-Naxx BiS would be the following setup :
Death Sting MH / Pugio OH
C'Thun Belt, ACLG (despite Death Sting, yeah), 3/8 T2 (Bracers, Pants, Chest), 3/5 T2.5, Accuria + MDR, Prestor, C'Thun cloak.
3/8 T2 bonus being the reason why you want to use T2 pants over T2.5 (although the difference is veeery small anyway) and T2 Chest over T2.5.
Regarding Trinkets, I haven't finished my tool to model trinket swapping in my spreadsheet, but my money would be on Earthstrike + BotS, given that you'll be able to use both at the same time if everything is Blizz-like (BotS not affected by the 1.10 dual trinket nerf for some reason).
Of course if target has less than 1200 armor after SA / CoR / FF, things might change a bit ofc. But the burst of BotS, wait for the 1200 armor pen (trinket lasts 30s so you got time) then ES + BF + AR is probably completely insane.
And then vanish + swap to DFT + HoJ after CDs ofc.

I think I mostly have figured that out as well but I wasn't sure if it was better to use snd/evis when I had 4 combo points or waiting till 5 and potentially wasting a combo point.
