What does +damage/heal do to a Paladin?

Since lots of theory-crafting is lost or inaccurate or not applicable to Nost, I would like to start a thread on which ability of a Paladin benefits for how much by +damage/+heal.

If you'd like to contribute to this list, please always include your sources (combat log results/theory-craft pages), so others can verify them. My main source so far is the TheoryCraft 1.06.5 addon, and some rudimentary testing in game. Values with a * are considered "not yet tested enough to be sure".
