I seem to remember first aid not costing money when you got to the levels that the trauma surgeon taught you (240+). It costs 10g for the Heavy Mageweave bandage and goes up 2.5g to learn Runecloth bandage. Goes up another 2.5g for Heavy Runecloth bandage.

I didn't know anything about it so i read some old vanilla comments. these posts seem to indicate they were free IF you did a certain quest with specific vendors that are listed in the thread. One person indicates better to do that then buy it from any other vendors in major cities as it costs a lot.
